ABCD Fuel Assistance Program Originally printed in December, 2004 Residents of Massachusetts may be eligible to receive up to $590 in fuel assistance through ABCD's Fuel Assistance Program which runs from November 1 to April 30. -Discounted rates for gas, electricity and telephone are applied automatically for all eligible recipients. -Residents with heat included in their rent may qualify for help with rent if they do not live in subsidized housing. -This federally-funded program serves low-income, elderly and handicapped residents in thousands of households every year. Proof of income for everyone in your household who is 18-years or older for the past 30 days. If someone is 18-years or older and is a full-time student, submit a letter from their school or college. 2. A Social Security number for everyone 18-years or older. 3. You need to provide one piece of identification for the person who is applying. The following list includes appropriate examples: -proof of TAFDC -proof of SSA/SSI -proof of Veteran's Benefits -proof of Pension/Annuity -proof of Mass Health card -copy of your Social Security card -a birth certificate -a pay stub with your SS number -a copy of driver's license -copy of state issued ID card -a copy of your Food Stamp card -a copy of Medicaid coverage card -a copy of your U.S. Military card 4. Most recent primary heating bill - oil, gas, electric PLUS... Renters/Heat Included In Rent: If your heat is included in your rent, we need: -a copy of your lease stating amount of rent and who is responsible for paying for heat and utilities, OR -a statement from your landlord stating that "Heat is included in the rent," the monthly rent amount, your address, and the name, address, telephone and signature of the landlord. Renters Who Pay For Their Own Heating Bill: You need to provide proof of home address by providing one of the following: -Copy of your current lease or a rent receipt, OR -by having your landlord complete the Renter Address Verification Form. Homeowners: Please submit a mortgage statement or a note stating why you have no mortgage.
